Does HMB Preserve Muscle During Weight Loss or Bed Rest?

HMB, short for beta-hydroxy-beta-methylbutyrate, is a metabolite of the amino acid leucine. The question of whether it protects muscle gets two different answers depending on why the muscle is being lost.

In bed rest, the signal is consistent. Deutz and colleagues in Clinical Nutrition in 2013 confined healthy adults aged sixty to seventy-six to complete bed rest for ten days in a randomised, placebo-controlled trial. The placebo group lost 2.05 kilograms of lean body mass; the HMB group lost 0.17 kilograms, a difference that was statistically significant. A 2015 meta-analysis by Wu and colleagues in Archives of Gerontology and Geriatrics pooled seven randomised trials in adults over sixty-five and found about 0.35 kilograms more muscle mass with HMB than control.

In intentional weight loss after bariatric surgery, the protection did not clearly appear. A 2025 trial by Botella Martinez and colleagues in Nutricion Hospitalaria compared an HMB-enriched supplement with a standard protein supplement in twenty-four patients for three months after surgery and found no significant difference in muscle preserved, fat lost or grip strength. A 2022 trial by Comas Martinez and colleagues in Nutrients compared three formulas in sixty gastric-bypass patients: the share of weight lost as fat-free mass was 34.6 percent with the HMB-enriched formula and only 19.1 percent with a short-peptide formula.

One trial complicates the split. Chang and colleagues in 2023 studied twelve collegiate boxers during a six-day weight cut and found fat-free mass held steady with HMB while it fell on placebo. That is a young, elite population on an extreme short protocol, very different from either setting above.

The biggest gap is the one most people mean when they ask: no retrieved trial tested HMB during ordinary, gradual, non-surgical dieting in older adults. Why the bed-rest effect did not replicate after surgery is unresolved. Anyone considering supplements around surgery or a period of immobility should raise it with a physician.
