What Does Tirzepatide Actually Do? What the Research Shows

Tirzepatide is a synthetic 39-amino-acid peptide that activates two gut-hormone receptors at once: GIP and GLP-1. Older drugs in the class, semaglutide, liraglutide and dulaglutide, act on GLP-1 alone. It is a prescription medication, FDA-approved as Mounjaro for type 2 diabetes in 2022, as Zepbound for chronic weight management in 2023, and for moderate-to-severe obstructive sleep apnea in adults with obesity in 2024.

The pivotal obesity trial is SURMOUNT-1, published in the New England Journal of Medicine in 2022. It randomized 2,539 adults with obesity or overweight and no diabetes to tirzepatide or placebo for 72 weeks. At the highest dose, mean weight reduction was 20.9% versus 3.1% with placebo, and roughly half of participants on the two higher doses lost 20% or more of body weight, versus 3% on placebo.

SURPASS-2, published in the same journal in 2021, compared tirzepatide directly with semaglutide in 1,879 adults with type 2 diabetes on background metformin over 40 weeks. All three tirzepatide doses produced significantly greater reductions in both HbA1c and body weight than the semaglutide dose used at the time.

What happens after stopping was tested directly. SURMOUNT-4, in JAMA in 2024, put 783 adults through a 36-week lead-in on tirzepatide, during which they lost a mean 20.9%, then randomized 670 to continue or switch to placebo for 52 weeks. The placebo group regained weight equal to 14.0% of body weight, while the group that continued lost a further 5.5%.

The limits: the evidence comes almost entirely from industry-run trials, the longest windows are under two years, and the pivotal obesity trials excluded anyone with a lifetime suicide attempt or a serious active psychiatric disorder, so they cannot answer questions now being asked about mood effects in that population. Thyroid C-cell tumors seen in rodents carry a boxed warning with unresolved human risk. Decisions about this medication belong with a physician.
